Pediatric and Adolescent Exercise and Nutritional Asessment /Intervention

The John Morrison White Clinic is a research, academic and service unit of the Lancaster campus of the University of South Carolina.  The well equipped cardiovascular and pulmonary laboratories, along with the nutritional and body composition assessment capabilities make the clinic unique within the USC System.  Numerous activities, projects and services are conducted in the Clinic by USCL faculty and staff, with the support of consulting physicians and other health care professionals in the Lancaster Community.  The Clinic is involved in cooperative programs with the USC Schools of Medicine and Public Health, with students from both schools working here throughout the year.


CLINICAL SERVICES

LANCASTER AREA CARDIAC REHABILITATION PROGRAM

In cooperation withthe South Carolina Vocational Rehabilitation Department, USC Lancaster provides a comprehensive program of  cardiac rehabilitation for Lancaster and the surrounding counties, including some in North Carolina.  The multidisciplined program is fully certified by the Joint Committee on Cardiovascular Rehabilitation of the South Carolina Medical Association and the South Carolina Affiliate of the American Heart Association.

The cardiac rehabilitation program consists of three phases of active participation by the patient in his or her recovery from heart disease.  Phase I takes place in the hospital and consists of the initial educational aspects of the program, progressive self-care and light physical activity.

Phase II is the first part of the program which is conducted in the J. M. White Clinic.  Patients should enter this phase as soon as medically feasible following discharge from the hospital.  The educational activities are expanded, as more emphasis is placed on the exercise portion of the program.  Phase II participants come to the clinic three (3) days per week for 12 weeks of medically prescribed and monitored exercise.  The cost of Phase II is reimbursed by many insurance carriers, including Medicare, within individual contract limitations.  A physician and an exercise physiologist supervise the program.  Registered nurses, registered dietitians and vocational counselors work with each patient to help him or her learn how to make any required changes in life-style.  Support is provided in the areas of nutrition and weight control, and in returning to work when appropriate.  Assistance with individual special needs can also be provided.  The ultimate goal of the program is to help each patient return to a healthy, active, high quality life.

Phase III, lasting 6 to 12 months following Phase II, is an option for those who want to continue in a supervised setting.  All of the services of Phase II are available, but the medical monitoring is not provided.  A fee is charged for participation in Phase II.

Physician Referral is required for participation in the cardiac and pulmonary rehabilitation programs. Medical records must also be provided.

PULMONARY REHABILITATION

Similar to the cardiac rehabilitation program, this service provides medically prescribed and monitored exercise therapy for patients suffering from various respiratory conditions.  Supplemental oxygen is available for use during exercise when requested by a physician.  The appropriate educational services are also available to pulmonary patients.  Fees are charged for this service.

Physician Referral is required for participation in the cardiac and pulmonary rehabilitation programs. Medical records must also be provided.


LABORATORY SERVICES

CARDIOPULMONARY EXERCISE TESTING

The J. M. White Clinic is equipped to conduct metabolic, cardiovascular and pulmonary exercise stress testing with state-of-the-art equipment.  This testing is required upon entry and exit for all cardiac and pulmonary rehabilitation patients.  The service is available to others when requested by a physician or otherwise indicated by accepted standards of medical practice.  All results will be interpreted by the Clinic Medical Director.  Reports will be sent to appropriate parties if requested by the patient.  Fees apply for these services.  Insurance forms will be filed upon request.

NUTRITIONAL AND BODY COMPOSITION ASSESSMENT

Computerized dietary assessment can be conducted in the Clinic.  The program data base is regularly updated in an attempt to maintain the validity of the assessment.

Body composition can be determined by appropriate anthropometric measurements and hydrostatic (underwater) weighing.  These remain the most reliable and validated methods available.

Reports of the results, with explanations and recommendations will be provided.  Follow-up services are also available.

Fees apply for these services.

CENTER FOR DEVELOPMENTAL EXERCISE AND NUTRITIONAL RESEARCH

The importance of physical activity and nutritional status during the life cycle has been given considerable attention recently, both in the media and by the scientific community.  Most of our current knowledge, however, is focused on the young adult and middle-aged segments of society.  There are a number of health related issues involving both children and older persons that remain open for investigation.  The J. M. White Clinic conducts an ongoing research program dealing with the mechanisms of exercise and nutrition as they relate to health status and age.  The purpose of the research aspect of the Clinic operation is to contribute to the existing knowledge in these specific areas.  Faculty from both the Lancaster and Columbia campuses of USC are involved in this research, as are graduate students from the Schools of Public Health and Medicine.

Research is an important part of our mission, especially as health issues are coming more and more to the forefront in every aspect of our lives.  From time to time, these research projects will require volunteer subjects.  Some of these projects have intervention services associated with them which offer direct benefit to participants, while others are strictly research based.
