Pre-Appointment Forms

We would like for you to complete Pre-Appointment Forms before your actual appointment. This provides us with some background information about you that will allow us more time during our initial session to get to know you and how to best provide you with support. 

There are several options about how to accomplish this:

• You can come 15 minutes prior to your Intake Appointment to fill them out in the waiting room;
• You can pick them up from the University of South Carolina Lancaster Counseling in advance of your appointment during normal working hours and fill them out at your leisure; or
• You can print and complete the necessary forms at home and bring them to your appointment.

The Pre-Appointment Forms include the following: 

1. The Intake Form that collects demographic, identifying information, information on past history, and your current situation.
   
   
2. The Informed Consent Form for services which provides information about our counseling service. Please print out and sign a second copy for you to keep.
   
   
3. The Notice of Privacy Practices (NPP). We are required by law (HIPAA) to give you this document and ask you to acknowledge receipt of the NPP.     ***Attached to the NPP is the Acknowledgment of Receipt of Notice of Privacy Practices (NPP, page 5). Please sign this form to verify that you have received this document. You are not required to read (or print) the NPP prior to your appointment.
